The Janitor from Scrubs: Sitcomdle character card

Who is the Janitor?

The Janitor is the hospital's custodian, and his feud with J.D. starts in the very first episode. He is trying to open a jammed door when J.D. suggests a penny might be stuck in it. There is one, and from that moment the Janitor is convinced J.D. put it there and vows revenge.

For eight seasons he is known only as the Janitor. In the season eight finale he finally tells J.D. his name is Glen Matthews, a nod to the janitor Flynn voiced on the cartoon Clone High. Given how freely he lies about everything else, fans still debate whether to believe him.

Personality and comedy

He is a master of elaborate, mean-spirited pranks, wildly unreliable stories about his past, and schemes involving his "brain trust" of hospital staff. He has a softer side, too: he gives J.D. a pass after J.D.'s father dies, and he has a soft spot for Elliot, whom he coaches through her season three makeover.

He also leads an air band, the Cool Cats, with Turk, Ted, and Lloyd the delivery man. The Janitor gets his own turn as narrator in season five's "His Story III," in which he locks J.D. in a water tower for the day and then quietly bonds with a patient who cannot communicate.

Key storylines and legacy

Neil Flynn was a recurring player in season one and became a series regular from season two on. In season eight the Janitor marries his girlfriend, Lady, and in "My Finale" he admits the truth about the penny: he saw J.D. accidentally drop it and was waiting for him to own up, a test of character J.D. failed.

Season nine says the Janitor quit the day after J.D. left, once he realized J.D. was not coming back, and Flynn appears only as a guest that year. He returns as a guest in ABC's 2026 revival, now a chief custodian for the Joint Commission, with a son working maintenance at Sacred Heart.
